Organise workshops to sensitize communities
Especially schools, churches, community based organisations, community groups and any other communities identified as essential for the programme activities
Conduct awareness raising activities
Such as community health events and radio talk shows in the local communities
Refer identified individuals suspected of having heart disease to medical resources for further follow up
Perform community outreaches to follow up people living with heart disease
Provide peer-to-peer counseling for people living with Heart disease.
Helping heart disease patients secure transport to medical appointments
Provide family counseling for PLWRHD
